Thank you for participating in the 22nd Annual Superstition Mountain Rotary Club Charity Golf Scramble at Gold Canyon Golf Resort! Please fill out this registration form to sign up for the event.

To fill out the online Registration Form, you will need to have a foursome. Gold Canyon Golf Resort will need all four players names, as well as one email and phone number for the team member who is signing up the foursome.

**If you do not have a foursome but still wish to participate in the event, please fill out a registration form in the Gold Canyon Golf Resort golf shop.**

The cost for the event is $165.00 per player ($660.00 per team). Please make checks payable to Superstition Mountain Rotary Club. You can turn in the check to the Gold Canyon Golf Resort golf shop, or mail them to P.O. Box 565, Apache Junction, AZ - 85117 by December 10th, 2021.
